bobbin thread comes loose

I will be stitching along just fine with my HQ 16 then all of a sudden the bobbin thread comes out from its tension holder. Obviously the stitches then are a mess. I have oiled, changed needles, changed bobbins, rethreaded, adjusted upper and bobbin tensions. Still will happen suddenly. Anything else I can do? I have used the same thread brand for over a year when this started happening.

How often does this happen? All quilting machines will have the bobbin thread slip out of the outside tension from time to time. I would love to be able to figure out a good solution for this. When I start getting bad tension all of the sudden, I know it is either that the thread has slipped out of the bobbincase tension or there is a ball of lint under that bobbincase tension.
